Insurance Policies
Our office is happy to assist you in completing your insurance claims. To do so, you must first provide us with either validated insurance claim forms or your insurance card. Unfortunately, due to the extensive variability of current policies and contracts, we are only able to provide you estimates, rather than guarantees, of your true coverage. Also, please be aware that balances not covered by insurance are still your responsibility to pay. It is for this reason we inform all insured patients to remain aware of their current benefits provided.

Financial Policies
Our office accepts cash, debit cards, Visa, Mastercard, and checks as acceptable forms of payment. For each appointment, the parent accompanying the child to the appointment will be responsible for payment
Cancellation Policy
If a cancellation is unavoidable, please call our office a minimum of 24 hours in advance so that we may give your child’s appointment time to another patient. Thank you.
