Insurance Policies

Our office is happy to assist you in completing your insurance claims. To do so, you must first provide us with either validated insurance claim forms or your insurance card. Unfortunately, due to the extensive variability of current policies and contracts, we are only able to provide you estimates, rather than guarantees, of your true coverage. Also, please be aware that balances not covered by insurance are still your responsibility to pay. It is for this reason we inform all insured patients to remain aware of their current benefits provided.

Financial Policy

Our office accepts cash, debit cards, Visa, Mastercard, and checks as acceptable forms of payment. For each appointment, the parent accompanying the child to the appointment will be responsible for payment.

Information Collected Online

Any information collected via our website is used to improve the content and advance our patient goals. We collect: domain name and internet protocol (IP) address of visitors to our website; aggregate information on what pages users access or visit; user-specific information on what pages users access or visit; and information volunteered by users, including from surveys, purchases and/or registrations. We also may collect the e-mail addresses of those who communicate with us via e-mail.

Your Rights

To help us protect your privacy, please contact us if you believe any information you provided us is being used for purposes other than those for which it was originally collected.

Children’s Privacy

If you are under 18 years of age, you should not provide information to the Tooth Engineering website without the involvement of a parent or guardian. Without parental consent, online information will be used only to respond directly to the child’s request and will not be used for other purposes.

Disclosures Related To Communications With You Or Your Family

We may contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you or relate specifically to your medical care through our office. For example, we may leave appointment reminders on your answering machine or with a family member or other person who may answer the telephone at the number that you have given us in order to contact you.

We may disclose your child’s protected health information to the provided list of parents or legal guardians involved in your child’s care. We will only disclose the protected health information directly relevant to their involvement in care or payment.

If a cancellation is unavoidable, please call our office a minimum of 24 hours in advance so that we may give your child’s appointment time to another patient. Thank you.
